Requests for Tenancy Approvals must include all of the following information in order to be accepted: Address of Unit (must include Apartment number where applicable and Zip Code) Requested Beginning Lease Date Number of Bedrooms Year Constructed Proposed Rent Security Deposit Amount Date Unit will be available for Inspection Type of Unit Utilities (must specify fuel type for Heating, Cooking and Water Heating) Utilities (must have notation in the provided by and paid by columns for each utility listed on form) Must have signature of the owner and phone number Must have signature of the Head of Household and phone number Proof of Ownership form must be completed (forms marked on file will not be accepted, for must have appropriate signature); also see below W-9 must be completed (forms marked "on file" will not be accepted, must also have signature)
